I think this because the bootstrap code in TSETCAT-.o reads,
literally:

<<TSETCAT-.lsp BOOTSTRAP>>=
@

Perhaps the .o file is being generated only on the second pass?

I believe this error no longer occurs in any version or fork of Axiom.
